Responsibilities:
Assessment of service needs, case management planning and coordination, collecting data and documentation in case records, revisions, problem resolution following NYS Health Homes and Medicaid standards, advocate for clients.
Knowledge of behavioral health diagnoses and symptomology, must be trauma informed.
Qualifications:
- A Master's degree in psychology, social work, or human services with 2-3 years relevant community-based experience.
- Previous experience in case management or a related field is highly desirable.
- Knowledge of medical terminology, physiology, and healthcare systems is beneficial.
- Strong communication skills, both written and verbal, to effectively interact with patients and healthcare professionals.
This position requires a compassionate individual who is dedicated to providing quality care to patients.
The successful candidate will have a strong understanding of case management principles and be able to effectively coordinate patient care.
